Unpacking the PEACH Trial: What is Levetiracetam’s Role in Spontaneous Intracerebral Hemorrhage?

BACKGROUND: intracerebral hemorrhage (ICH) makes up 10-15% of all strokes and significantly contributes to overall stroke-related morbidity and mortality  (Hostettler 2019). Levetiracetam is frequently used to prevent seizures in patients with conditions such as ICH, traumatic brain injury (TBI), supratentorial neurosurgery, and subarachnoid hemorrhage (SAH). However, guidelines offer no strong recommendations for its use, specifically in spontaneous ICH (Fang 2021). Ongoing debates persist regarding the drug’s effectiveness, optimal dosage, and potential side effects. About 30% of patients experience early seizures within seven days following the onset of a stroke  (Peter-Derex 2022). While clinical seizures are readily identifiable, subclinical seizures—which comprise most seizures after a hemorrhage—are hard to detect. The long-term impact of these subclinical seizures is still unclear. More research is needed to inform preventive treatment strategies for seizures in patients with spontaneous intracerebral hemorrhage (ICH).
