The nasopharyngeal airway is an airway intervention named in TCCC guidelines, but the evidence-based device list names specific products rather than the generic adjunct, and no manufacturer or model is identified for the airways in this bundle.

Scope of practice. Airway sizing, gag-reflex assessment and adjunct selection are clinical judgments that require trained providers. The nasal route is avoided where a basilar skull or midface fracture is suspected; the oral route is contraindicated in a patient with an intact gag reflex. What is included in the Basic Airway Pair bundle?

Is the nasopharyngeal airway (NPA) a CoTCCC-recommended item?

Yes. The nasopharyngeal airway (NPA) is a CoTCCC-recommended airway adjunct for casualties who are unconscious or semi-conscious. It is specifically recommended for situations where an oropharyngeal airway (OPA) cannot be used—such as in patients with a present gag reflex or suspected jaw injury—making it a standard component of military and tactical IFAK builds.

When should I use the NPA versus the Berman OPA?

Use the NPA (28Fr) for semi-conscious casualties who have a gag reflex or where you cannot open the mouth, and when basilar skull fracture is not suspected. Use the Berman OPA for deeply unconscious casualties with no gag reflex, where tongue obstruction of the pharynx is the primary concern. The Berman OPA's open-channel design also permits direct suctioning through the device.

Is the 28Fr NPA the right size for most adult casualties?

The 28Fr NPA is the most commonly carried size in IFAKs and combat medical kits for adult casualties and is considered a practical standard sizing for field use where exact measurement is not possible. It is appropriate for most average-to-large adult patients. If your kit protocol calls for multiple sizes, consider supplementing with additional NPA sizes separately.
