XSTAT P30 Wound Dressing

The high-capacity XSTAT applicator: a 30 mm barrel carrying roughly 108 compressed mini-sponges in three porous removable pouches. On blood contact the sponges expand to as much as ten times their compressed size in 15 to 20 seconds, filling a deep wound cavity from the inside. Every sponge carries an X-shaped radiopaque filament, and the pouches are what make surgical retrieval tractable.

Is the XSTAT P30 CoTCCC recommended?

Yes. The XSTAT P30 (XSTAT 30 Pouch) is CoTCCC-recommended for the control of severe, life-threatening hemorrhage from junctional wounds in the groin or axilla not amenable to tourniquet application, and from narrow entrance extremity wounds in the arms or legs. It is FDA cleared (K210676) and currently fielded by the U.S. Military. The XSTAT is the only injectable hemostatic device CoTCCC-recommended for pre-hospital internal use.

What is the key difference between the XSTAT P30 and the older XSTAT 30 Gen 2?

The XSTAT P30 (Pouch) is the current generation, cleared under FDA 510(k) K210676. Its key innovation is that the approximately 108 mini-sponges are now enclosed in three separate porous polypropylene pouches within the applicator. This makes surgical removal faster and more organized — the surgical team can retrieve sponges in three grouped pouches rather than individually. The P30 also has less packaging and a smaller cube space than the XSTAT 30 Gen 2.

When should the P30 be used instead of the P12?

Use the XSTAT P30 for larger wounds — specifically large exit wounds (gunshot), blast injuries with significant wound cavitation, and junctional wounds (groin/axilla) where the wound cavity is large enough to accommodate the 30 mm diameter applicator. Use the XSTAT P12 for narrow entrance wounds and smaller penetrating wounds where the 30 mm P30 cannot be inserted. When in doubt, attempt the P30 first for junctional wounds; if the applicator won't fit, use the P12.

How many sponges does the XSTAT P30 contain?

The XSTAT P30 contains approximately 108 compressed mini-sponges per applicator, organized into three porous pouches of roughly equal sponge count. Each sponge expands up to 10 times its compressed volume within 15–20 seconds of blood contact. Each sponge absorbs approximately 3 mL of blood, giving the P30 an approximate total absorption capacity of ~324 mL per applicator. For severely injured patients with large wound cavities, multiple P30 applicators may be required.

What must the surgical team know before removing XSTAT P30 sponges?

All XSTAT P30 sponge pouches must be removed intraoperatively by a surgeon with the capability for proximal and distal vascular control — this means an operating room with standard surgical tools, not a field medical facility. The three sponge pouches are visible on X-ray via the radiopaque filaments on each pouch and each individual sponge. A post-removal radiograph is required before wound closure to confirm complete sponge retrieval. Providers must document XSTAT P30 use and the number of applicators on the casualty card and communicate clearly to the receiving surgical team.
