Vest space is the real constraint. Nesting the medical insert behind the magazines buys the panel back — as long as the wearer can find the pull without looking.
At a glance
10 things to know about the 3 Magazine Rapid Access Pouch
What 6:8 built it to do, what it holds, and what it leaves for you to supply.
-
01
Why it exists
6:8 designs the Rapid Access Pouches to minimize the number of pouches on a tactical MOLLE vest or battle belt by nesting the medical items needed for self and buddy aid with other critical items, such as weapon magazines, in one pouch.3
-
02
Stacked and flat-packed
The profile is kept down by a stacked, flat-packed configuration that places IFAKs and blood kits in a readily accessible position, actioned with a quick pull.3
-
03
Three magazines
The pouch holds three standard 5.56 mm 30-round rifle magazines.3 Magazines are not included — 6:8 lists them as sold separately.3
-
04
An insert that leaves with the casualty
An inner insert straps to the leg and carries the end items contained in an IFAK.3 Taking it off the vest and onto the casualty's leg keeps the supplies with the person who needs them.
-
05
A detachable tourniquet pouch
A detachable Rapid Access tourniquet pouch holds a Tactical Mechanical Tourniquet (TMT) and a TQ-RAM, with its own quick-pull design.3 Mount it wherever you can reach it with either hand, without looking.
-
06
The tourniquet is not part of the pouch
The Evidence-Based Standard Devices & Adjuncts list names the Tactical Mechanical Tourniquet, NSN 6515-01-656-6191.2,6 That status is the tourniquet's alone — the pouch is not on the list. The TMT and the TQ-RAM are not included with the pouch.
-
07
Built for mittens
6:8 designed the quick pulls to be actionable with extreme-cold mittens or other gloves.3 Cold hands do not do fine motor work; a pull you can find with a mitten is the difference between a carried tourniquet and a deployed one.
-
08
MOLLE weave backing
The pouch attaches to PALS webbing through a MOLLE weave backing, so it mounts to a plate carrier, vest or battle belt.3 It measures 12.5 × 7.5 × 1.5 in. and weighs 11 oz.3
-
09
Room for shears or a blanket
The design also holds a small pair of trauma shears or an emergency blanket.3
-
10
Load the insert to the wearer's level of care
The pouch decides where things sit; the wearer's training decides what goes in the insert. A tourniquet and a pressure dressing suit nearly everyone; a decompression needle or a surgical airway belongs only with a provider trained and authorized to use it. Carrying a tourniquet is not training — practice drawing it from this exact pouch, gloved.
How to set it up and use it
Load it to your level of training, mount it, then drill it.
-
1Load the magazines. Seat three standard 5.56 mm 30-round magazines in the magazine section. Magazines are supplied separately.3
-
2Load the insert. Pack the IFAK end items you are trained to use into the leg-strap insert. Keep a tourniquet out of the insert and in the tourniquet pouch, where you can reach it first.
-
3Stage the tourniquet. Place a Tactical Mechanical Tourniquet, with a TQ-RAM if you carry one, in the detachable RAP pouch and mount it where either hand reaches it.3 The TMT is stored in its one-handed configuration.4,5
-
4Mount the pouch. Weave the MOLLE backing onto PALS webbing on the plate carrier, vest or battle belt.3
-
5Choose the tourniquet placement. Under threat, or when you cannot see where the blood is coming from: hasty — high and tight, over clothing. When the source is found and time and safety allow: deliberate — on bare skin, 2–3 in. above the wound. If that point lands on a joint, move above the joint. Never place it across or directly over a joint.1
-
6Pull, not dig. Pull the tab for the tourniquet pouch or the medical insert. Take the TMT, hold the strap end by the white time-label tab, let it fall open with "PULL" facing away from you, then ratchet, twist, lock and record the time.1,4
-
7Take the insert with the casualty. Release the insert and strap it to the casualty's leg so the supplies travel with them.3
Where it fits
- Carrying three 5.56 mm 30-round magazines and a medical kit on one panel3
- Cold-weather and mountain work, where quick pulls must open with mittens3
- Keeping the medical insert with the casualty on a leg strap3
- Staging a TMT and a TQ-RAM in a detachable pouch3
- Carrying a tourniquet model other than the TMT — the tourniquet pouch is documented for the TMT and TQ-RAM3
- A full medical loadout — it is an insert and a tourniquet pouch, not a bag
- Casualty care beyond your training — load only what you are trained to use
- Children's care — most pediatric limb bleeding needs firm direct pressure, not a tourniquet
The evidence
The evidence below is for the tourniquet the pouch is built to carry — what each study tested and how strong it is.
| Study | Evidence type | What was studied | What it found |
|---|---|---|---|
| Kragh et al., Ann Surg 2009 | Human · combat clinical | Tourniquets as a class: 232 casualties with 428 tourniquets, Baghdad, 2006 | Survival 90% when tourniquets were applied before shock vs 10% after; no limb lost because of a tourniquet |
| Beaven et al., J Spec Oper Med 2018 | Human volunteer · randomized | 24 British military volunteers self-applying the C-A-T and the TMT at mid-thigh, ultrasound of popliteal flow | Success C-A-T 92% vs TMT 71% (not significant); median time to occlusion 37.5 s vs 35 s |
| Treager et al., J Trauma Acute Care Surg 2021 | Human volunteer · randomized crossover | C-A-T Gen 7 vs TMT vs SOFTT-W on arm and leg, Doppler pulse, then retested after crawling and dragging | Arm failure 5.6% vs 19.4% vs 58.3%; leg failure 27.8% vs 44.4% vs 61.1%; no difference after movement |
| Montgomery et al., J Spec Oper Med 2019 | Expert review · evidence-based update | Review of limb tourniquets for the evidence-based's 2019 update, which added the TMT | evidence-based voted to update its list in April 2019; the authors note the lack of unbiased data and official test criteria |
Strongest to weakest: human clinical data, then human-volunteer tests, then expert review. Every study tested a tourniquet; none tested the pouch, and the TMT scored numerically below the C-A-T Gen 7 in both volunteer trials.
How it compares
How the pouch differs from carrying a tourniquet on its own.
| 3 Magazine RAP | Rapid Access Tourniquet Pouch | Single-purpose IFAK pouch | |
|---|---|---|---|
| Carries | 3 rifle magazines, a medical insert and a detachable tourniquet pouch | One tourniquet | Medical supplies only |
| Rifle magazines | Three 5.56 mm 30-round, magazines sold separately | No | No |
| Medical insert | Detachable, straps to the leg | No | Fixed |
| Tourniquet carried | TMT and TQ-RAM, in a detachable pouch | Sized for wider tourniquets such as the TMT | Varies |
| Mounting | MOLLE weave backing | MOLLE/PALS weave backing | Varies |
| evidence-based standard | The TMT it holds — yes; the pouch — no | The tourniquet carried — only if it is on the list | Not applicable |
Pouch details from 6:8 Medical Solutions.3 The evidence-based list names limb tourniquets, never a pouch.2
