Anything you have to unfasten first is something you can fail to unfasten under stress. A friction case has one motion: grab and pull.
At a glance
10 things to know about the SOF® + RIGID TQ Case® combo
How the case carries the tourniquet, what changed in the fifth-generation SOF®, and what the options mean.
-
01
Two makers, one carry system
The tourniquet is TacMed's SOF® Tourniquet, fifth generation; the case is Eleven 10's RIGID TQ Case® for the SOF®TT / SOF®TT-W, belt version, part E10-3011B. TacMed states the case holds any SOF® Tourniquet.3,6,8,10
-
02
Friction, not flaps
The case holds the tourniquet by friction, so there are no straps, buckles or flaps to work. Grab the exposed part and pull: a one-handed, gross-motor draw that works with wet or bloody hands.8
-
03
Injection-molded, not Kydex
The original version of Eleven 10's case was Kydex; the RIGID TQ Case® is injection-molded nylon, with a two-piece body that drains water out of the bottom, a textured front panel and a PVC medical cross patch.8,9
-
04
A TEK-LOK belt mount
The combo ships on a Blade-Tech TEK-LOK that fits duty belts up to 2.25 in. The case mounts vertically, horizontally or inverted, with adjustable ride height.8,9
-
05
Other mounts
Swap the TEK-LOK for a Tactical Tailor Malice Clip to run it on MOLLE, or mount it in the accessory position of a Safariland drop-leg shroud or on a Raven Concealment ModuLoader — hardware sold separately.8,9
-
06
A wedge that shows when the slack is gone
The SOF® Tourniquet's 1.5 in. polyester strap carries a contrasting slack indicator wedge under the windlass; when it shows, the slack is out and fewer windlass turns are needed.3 In a manikin study of an earlier SOFTT-W, failed applications carried 10.5 mm of slack against 3.2 mm in successful ones.13
-
07
Two-stage lock, trapped-limb buckle
The TRAC clip holds the windlass for a moment; the Tri-Ring Lock™ locks it. If a limb is pinned, the buckle unclips, goes around and reconnects.3,4
-
08
The evidence on the SOF design is mixed
Earlier SOFTT-W generations trailed the C-A-T in volunteer and manikin testing — 58% versus 6% failure on the arm, 35% versus 68% hemorrhage control on a manikin leg.12,13 In a 2023 live-thigh study that included this fifth generation, 1 of 64 Gen 5 applications was not correctly secured, against 47 for the C-A-T Gen 7.11
-
09
Five tourniquet colors
Black (84-0009), Rescue Orange (84-0010), Tan (84-0012), Red — the Red Cross version (84-0013) — and Trainer Blue (84-0011).3
-
10
Blue is a trainer
The Blue option is TacMed's Trainer Blue, a training device. In this case it is the right tool for practicing the draw from your belt — and it is never staged for duty use. Carry a Black, Rescue Orange, Tan or Red unit.3

-
1Decide where it goes. Under threat, or when you cannot see where the blood is coming from: hasty — high and tight, as far up the limb as it goes, over clothing. When the scene allows and you can identify the source: deliberate — on bare skin, 2–3 in. above the wound; if that point lands on a joint, move above the joint. Never across or directly over a joint.1
-
2Draw it. Grab the exposed part of the tourniquet and pull it straight out of the case.8
-
3Slide it on. Slide the loop over the hand or foot and up the limb above the injury. If the limb is trapped, unclip the buckle, pass the band around and reconnect it.4
-
4Pull the tail tight. Pull hard until the strap is snug and the slack indicator wedge shows the slack is out.3,4
-
5Twist and lock. Turn the windlass until the bleeding stops and the pulse below the tourniquet is gone — it hurts, and that is expected — then lock it in the Tri-Ring Lock™.1,4
-
6Still bleeding? Add a second. Tighten further; if one tourniquet does not stop the bleeding, apply a second one side-by-side with the first.1
-
7Write the time. Mark it on the tag and on the casualty card.1,4
-
8Reassess when you can. To convert a hasty tourniquet, put a second one on bare skin 2–3 in. above the wound, tighten it, then slowly release the first. If you still cannot identify the source, it stays high and tight.1,15
Where it fits
- Life-threatening bleeding from an arm or a leg
- A traumatic amputation, even one that is barely bleeding — adrenaline constricts the arteries, and when it wears off they open and the bleeding starts
- Belt carry beside a holster, radio and cuffs — the TEK-LOK fits belts up to 2.25 in.8
- A one-handed draw with wet or bloody hands8
- Junctional wounds — the groin, armpit and neck — where a limb tourniquet cannot be placed; pack them with a hemostatic dressing and hold pressure1
- Placement across or directly over a joint
- Bleeding that firm direct pressure or a pressure dressing controls
- Duty carry of the Blue option — it is a trainer
The evidence
The studies of the SOF design, which generation each one tested, and how strong the evidence is.
| Study | Evidence type | What was studied | What it found |
|---|---|---|---|
| Wall et al., J Spec Oper Med 202311 | Human · live-thigh volunteers | 64 appliers each applied 8 tourniquets, including the SOF Gen 5, SOFTT-W Gen 3, C-A-T Gen 7 and TMT, to live thighs | Not correctly secured: 1 SOF Gen 5 application vs 47 C-A-T Gen 7 and 31 TMT |
| Treager et al., J Trauma Acute Care Surg 202112 | Human · randomized crossover | C-A-T Gen 7 vs TMT vs SOFTT-W (earlier generation), arm and leg, volunteers, Doppler endpoint | Arm failure 5.56% C-A-T, 19.44% TMT, 58.33% SOFTT-W; leg 27.78%, 44.44%, 61.11%; after a crawl and drag, no significant difference |
| Katsnelson et al., Mil Med 202013 | Manikin · randomized | C-A-T Gen 7 vs SAM XT vs SOFTT-W (earlier generation), HapMed leg trainer, 60 military medicine cadets | Hemorrhage control 67.7% C-A-T, 73.3% SAM XT, 35% SOFTT-W; more slack with the SOFTT-W, and slack tracked failure |
| Kragh et al., J Trauma 200814 | Human · combat clinical series | 232 casualties, 428 tourniquets, Baghdad combat support hospital, 2006 | No limbs lost because of tourniquet use; tourniquet time not associated with increased morbidity |
Strongest to weakest: clinical series and live-volunteer studies, then manikin studies. Only the 2023 Wall study tested this fifth-generation tourniquet; the comparisons that favored the C-A-T tested earlier SOFTT-W generations. No study has tested the case itself.
