CRO Medical Pelvic Binder

A flat-packing pelvic binder closed by a BOA® precision dial instead of a buckle or ratchet. One hand turns the dial until it stops turning, which the manufacturer states delivers at least 33 lb (150 N) of circumferential force. The CoTCCC's 31 January 2017 update added pelvic binding under the Circulation component of TCCC. TCCC directs that a binder be applied for suspected pelvic fracture; it does not name specific binder products, and no pelvic binder holds a CoTCCC-recommended designation. Recognising the injury pattern, deciding to bind, and every subsequent intervention are clinical judgements made by a trained responder under their protocols.

Is any pelvic binder CoTCCC-recommended?

No. The CoTCCC does not designate specific pelvic binder products, and no pelvic binder carries a CoTCCC-recommended designation. What TCCC does direct is the intervention: the January 31, 2017 update added pelvic binding under the Circulation component, citing that 26% of service members who died in OEF/OIF had an associated pelvic fracture, and the current TCCC Guidelines effective 01 May 2026 retain pelvic binder application for suspected pelvic fracture. The CRO Pelvic Binder is an FDA-cleared device that meets the clinical requirement for circumferential pelvic ring stabilization, and it is available on DoD procurement vehicles including ECAT and GSA Schedule. How do I know if I've applied enough pressure with the BOA dial system?

The BOA dial system is specifically engineered to eliminate this uncertainty. Simply tighten the dial until you physically cannot tighten it further — that resistance point corresponds to a force of at least 33 lb (150N), which is the minimum required for effective pelvic stabilization. There are no documented cases of over-reduction using this system in existing clinical practice guidelines, so applying maximum achievable tightening force is the correct technique. No measurement, gauge reading, or second rescuer is required.

Does the CRO Pelvic Binder fit all adult patients?

The standard size accommodates hips from 32 inches to 50 inches, which corresponds to approximately 95% of the adult population. This coverage means a single standard binder addresses the vast majority of patients without needing to size-match before application — an important advantage in tactical and mass casualty environments where time and cognitive load are constrained. The compact flat-pack format makes it practical to carry one binder in a first-line aid bag or MARCH belt pouch.

Where exactly should the CRO Pelvic Binder be positioned on the patient?

The binder should be positioned at the level of the greater trochanters — not the iliac crest. Placing a pelvic binder too high (at the iliac crest) fails to reduce the pelvic ring and may be less effective at controlling hemorrhage. Landmark the greater trochanters (the bony prominences on the sides of the upper thigh) and center the binder at that level. This positions the device to apply circumferential force at the point that most effectively reduces open-book and lateral compression pelvic fractures.

How does the CRO Pelvic Binder compare to the SAM Pelvic Sling?

Both are pelvic stabilization devices with different mechanisms. The CRO Pelvic Binder uses the BOA dial system for one-handed tightening with a definitive force endpoint — the operator tightens until resistance stops, removing the subjectivity of buckle-based systems. The SAM Pelvic Sling uses a self-locking buckle with color-coded tension indicator. The CRO binder is approximately half the cube size of many competing binders and packs flat, making it easier to integrate into first-line kit configurations. Neither carries a CoTCCC product designation. Choice may depend on unit protocol and individual training familiarity.
