What is the i-gel Resus Pack? A second-generation supraglottic airway (SGA) from Intersurgical for EMS and emergency resuscitation. Made from medical-grade thermoplastic elastomer (TPE), the i-gel forms a non-inflatable anatomical seal of the pharyngeal and laryngeal structures without cuff inflation. Available in Size 3 (30–60 kg), Size 4 (50–90 kg), and Size 5 (90+ kg). Each pack includes the i-gel, lubricant sachet, airway support strap, and 12Fr suction tube. Supraglottic airway devices are advanced airway tools. What It Is
The i-gel is one of the most clinically studied supraglottic airway devices in emergency medicine. Launched in 2007, it uses a non-inflatable cuff of soft thermoplastic elastomer that molds to the perilaryngeal anatomy on insertion — creating a seal without inflating a cuff balloon. That removes one step and one failure mode compared with inflatable-cuff SGAs, and the gel-like material conforms to individual anatomy, reducing pressure-related mucosal trauma.
The Resus Pack is the EMS/emergency version, distinguished by protective cradle packaging (which maintains the device's shape in transit and serves as a lubrication base), the lubricant sachet, an airway support strap, and a 12Fr suction tube. The suction tube passes through the integrated gastric channel, allowing gastric decompression without removing the airway — important for reducing aspiration risk during prolonged resuscitation.
Size selection is by patient weight: Size 3 (30–60 kg), Size 4 (50–90 kg), Size 5 (90+ kg), each color-coded for rapid identification. The airway support strap secures the device during resuscitation and transport, resisting displacement during CPR compressions and patient movement.
The 2024 CoTCCC airway update evaluated EGA versus BVM ventilation in combat trauma and found SGAs were not superior to — and may be inferior to — BVM for non-cardiac-arrest combat trauma patients. Without RSI, EGA placement requires a deeply comatose patient. CoTCCC continues to support BVM as the primary ventilation strategy, with EGA as an option in specific circumstances authorized by the medical director.
