Standard pre-hospital practice is that an impaled object is stabilised in place rather than removed in the field, because removing it can release tamponade and cause catastrophic haemorrhage. Whether that applies to the casualty in front of you, and what to do instead, is governed by your training, your scope of practice and your medical direction — not by this page. Movement during extrication and transport is what a stabilisation device is for; it is not a substitute for haemorrhage control, and it does not change the priority order of the assessment.

Should impaled objects be removed in the field?

No. The standard of care in pre-hospital and TCCC settings is to stabilize impaled objects in place — do not remove them. Removing an impaled object in the field can cause catastrophic hemorrhage by disrupting tamponade effects, injuring blood vessels that have formed around the object, and releasing compressed tissue planes. The object should be secured, the wound dressed around it, and the patient transported to a surgical facility for operative removal under controlled conditions.

What sizes of impaled objects does the IM-PAL accommodate?

The IM-PAL accommodates objects with diameters from 0.25 inches to 1.25 inches. This range covers the majority of real-world penetrating objects encountered in pre-hospital trauma: knives, scissors, screwdrivers, rebar, metal shards, and similar materials. Objects outside this range — very thin wires or extremely large implements — may require alternative or supplementary stabilization approaches.

Why is a purpose-built device better than improvised impaled object stabilization?
