What It Is
The Pulmodyne VT Select advances BVM design by directly addressing the most common BVM failure mode: provider hyperventilation. Research consistently shows that with standard adult BVMs (1,900 mL bag), providers over-squeeze and deliver tidal volumes of 800–1,000 mL at rates above 20 BPM — far above the AHA-recommended 500–600 mL at 10 BPM. Hyperventilation reduces cardiac output, raises intrathoracic pressure, and is associated with worse cardiac-arrest outcomes.
The Select Valve addresses this mechanically: when activated, it restricts bag refill to ~4 seconds, so the rate cannot exceed ~10 breaths per minute regardless of how fast the provider squeezes. Activated mode delivers ~530 mL (one hand) to ~690 mL (two hands) per breath, keeping both rate and volume in therapeutic range without constant self-correction.
The integrated dial manometer allows continuous airway-pressure monitoring during manual ventilation — useful with lung disease, pulmonary contusion, or suspected pneumothorax. The adjustable PEEP valve can apply 5–10 cm H₂O of positive end-expiratory pressure when indicated, and the bacterial/viral filter protects patient and provider. The 1,200 mL bag is intentionally smaller than standard adult BVMs, further reducing hyperinflation risk.
The AHA recommends 500–600 mL tidal volume at ~10 BPM for adults. The 2024 CoTCCC airway update supports BVM ventilation as a first-line approach in Tactical Field Care and notes that correct technique (small tidal volumes) is superior to advanced-airway placement in many scenarios.
