What It Is
Lower-extremity injuries — gunshot wounds and severe lacerations to the thigh and lower leg — represent a disproportionate share of combat and tactical trauma. The femoral artery in the upper thigh can produce fatal exsanguination in under two minutes, and packing a deep thigh wound to the bone is one of the most technically demanding skills in the TCCC curriculum. This trainer is built to develop all three primary lower-extremity interventions — tourniquet application, wound packing, and pressure-bandage application — on one realistic platform.
The leg carries three distinct wound patterns. The severe laceration is the primary tourniquet and pressure-bandage station: trainees place a tourniquet at the optimal proximal location for the wound, simulate pulsatile flow by pressurizing the IV bag, and assess whether the application eliminates the pulsatile quality. The two gunshot wounds provide packing stations where trainees practice deep packing to the base of the cavity, making contact with the simulated vessel — a technique many practice too superficially. The internal bone structure, palpable through the tissue analog, supplies the tactile cue that “packing to the bone” requires and that no flat diagram can teach.
The self-healing material allows wounds to be repacked repeatedly without degradation, and the gravity-fed bleeding system needs no power. After each repetition the cavity is cleared and reset, the bag refilled, and the trainer is ready for the next trainee. The included hard case protects the trainer in transport to field sites.
