What wound types is the ITClamp® best suited for?

The ITClamp® is specifically designed for wounds that cannot receive tourniquet application — junctional and truncal wounds including neck/cervical wounds, groin and femoral triangle injuries, axilla and subclavian wounds, and large scalp lacerations. By mechanically approximating wound edges with micro-locking teeth and creating a stable hematoma under the clamp, it provides rapid bleeding control in anatomic locations where direct pressure is difficult to maintain and traditional packing techniques are hard to sustain during patient movement or transport.

How does the ITClamp® work differently from wound packing with gauze?

Wound packing with gauze works by filling the wound cavity with hemostatic material and applying sustained direct pressure to promote clotting — effective but requiring continuous manual pressure maintenance or securing with a dressing. The ITClamp® works mechanically by clamping the wound edges closed with a series of micro-locking teeth, creating a contained hematoma space where pressure builds naturally to arrest hemorrhage. The ITClamp can be applied in seconds without sustained manual pressure, making it particularly valuable when the medic must treat multiple casualties or maintain other interventions. The two NAR gauze packets in the bundle allow wound packing if the ITClamp cannot be applied due to wound geometry.

Is training required to use the ITClamp®?

Yes. The ITClamp® is an advanced hemorrhage control device designed for trained responders — tactical medics, military medical personnel, and EMS providers — who have received specific instruction in its application techniques and indications. The device is not intended for untrained civilian bystander use. Proper training covers wound type assessment, ITClamp placement technique, and post-application monitoring. Why does the bundle include two packs of NAR Wound Pack Gauze rather than one?

Two packs of NAR Wound Pack Gauze provide redundancy for several scenarios: managing two simultaneous wound sites in multi-casualty or multi-wound situations; repacking a single wound if the first pack becomes fully saturated before a stable clot forms; and staging one pack for use with the ITClamp and a second for an additional wound. In advanced hemorrhage management, having surplus gauze available is standard practice — a single pack may be insufficient for large-cavity or high-flow wounds, and having two packs eliminates the risk of running out during active hemorrhage control.

Where does this bundle fit in the MARCH protocol?

The Wound Clamp Pack Duo is specifically a Massive Hemorrhage (M) intervention tool, targeting the highest-priority step in the MARCH protocol. It addresses compressible hemorrhage from wounds not manageable with a tourniquet — complementing a tourniquet-capable IFAK rather than replacing it. For complete MARCH coverage, this bundle should be carried alongside an IFAK with tourniquet (for extremity hemorrhage), airway adjuncts (for Airway management), and chest seals (for Respiration/penetrating thoracic trauma).
