PRODUCT INFORMATION

ChitoGAUZE XR2 PRO by Tricol Biomedical (HemCon) is the next-generation chitosan hemostatic gauze featuring dual embedded X-ray detectable strips, a uniform chitosan coating on a polyester/rayon blend gauze base, and CoTCCC-recommended status. The 3 in. × 4 yd. Z-fold vacuum-packed gauze uses Chito+ technology — chitosan muco-adhesion independent of the clotting cascade — and offers antimicrobial properties against 26 bacterial organisms including MRSA. NAR Item #: 30-0265.

Product Overview

ChitoGAUZE XR2 PRO is the most advanced ChitoGAUZE product — an evolution of the original HemCon ChitoGauze that adds a second X-ray detectable strip and an optimized chitosan coating while maintaining the same 3 in. × 4 yd. Z-fold format used across the ChitoGAUZE product family. The designation "XR2" indicates two radiopaque threads embedded in the gauze, providing enhanced imaging reliability compared to the single-strip XR PRO version. The dual strips ensure that surgical teams can identify all gauze material in a wound cavity on X-ray, an important safety feature for packed wounds receiving surgical care.

The Chito+ technology coating provides a robust, uniformly applied chitosan layer on both sides of the polyester/rayon blend non-woven medical gauze. The positively charged chitosan interacts with the negatively charged membranes of red blood cells — an electrostatic muco-adhesion mechanism that is independent of the body's clotting cascade. This means ChitoGAUZE XR2 PRO maintains hemostatic efficacy in hypothermic patients, anticoagulated patients, and those with trauma-induced coagulopathy where Factor XII-dependent pathways (activated by kaolin in QuikClot) may be impaired. ChitoGAUZE XR2 PRO was added to the CoTCCC recommendations in the 2014 TCCC Guideline Change 13-05, alongside Celox Gauze, as a recommended alternative to QuikClot Combat Gauze.

A clinically important additional benefit is ChitoGAUZE XR2 PRO's antimicrobial properties. The chitosan coating demonstrates activity against a broad spectrum of 26 bacterial organisms — including methicillin-resistant Staphylococcus aureus (MRSA), Enterococcus faecalis, and Acinetobacter baumannii — organisms commonly encountered in combat and trauma wound environments before antibiotic treatment begins or transfer to a medical treatment facility is complete. ChitoGAUZE XR2 PRO can remain in place for up to 48 hours, longer than most hemostatic gauze dressings, and is removed by peeling gently from the wound without irrigation required (the non-shedding chitosan coating prevents residual material in the wound). The gauze conforms to complex wound geometries, filling irregular surfaces and penetrating wound tracks.

How to Apply

  1. Identify and expose the source of bleeding.
  2. Open the vacuum-sealed pouch and remove the Z-fold gauze.
  3. Cover the source of bleeding with the dressing; apply direct digital pressure to the source.
  4. Pack the dressing completely into the wound track, ensuring contact with all bleeding surfaces.
  5. Apply firm pressure for a minimum of 3 minutes (longer for arterial wounds). Do not lift to inspect — maintain continuous pressure.
  6. More than one dressing may be required for large wounds. If trimming the dressing, ensure both radiopaque elements are included in the applied portion.
  7. Secure with a pressure bandage. Can remain in place for up to 48 hours.

XR2 PRO vs. XR PRO vs. Combat Gauze

ChitoGAUZE XR2 PRO vs. XR PRO: Both are 3 in. × 4 yd. CoTCCC-recommended chitosan gauze with the same Chito+ mechanism. The XR2 adds a second X-ray detectable strip — a surgical safety upgrade for hospitals and combat support hospitals receiving packed wounds. Versus QuikClot Combat Gauze: Combat Gauze activates Factor XII (requires intact clotting cascade); ChitoGAUZE acts independently (works in anticoagulated, hypothermic patients) and adds antimicrobial benefit. The antimicrobial advantage and dual X-ray strips make XR2 PRO the preferred choice for prolonged field care and hospital handoff.
