M — Massive Hemorrhage
Stop life-threatening external bleeding first. Tourniquets for extremities, wound packing with hemostatic agents for junctional wounds. This is the number one preventable cause of preventable battlefield death.

Establish and maintain a patent airway. Nasopharyngeal airways, surgical airways, and recovery position techniques to prevent airway compromise in the casualty.

Identify and treat tension pneumothorax, open chest wounds, and flail segments. Chest seals, needle decompression, and monitoring for respiratory compromise drive this phase.

Control remaining bleeding, establish IV/IO access, and prevent hypovolemic shock. Fluid resuscitation, blood products, and tourniquet conversion when appropriate fall under this phase.

Prevent and treat hypothermia, manage head injuries and pain. Hypothermia triad of death compounds blood loss. Wrap casualties early, monitor mental status, and address pain control.
