Respiration: Three Interventions for Thoracic Trauma

The R in MARCH addresses breathing and thoracic injuries. Tension pneumothorax is a leading cause of preventable death in penetrating trauma -- air enters the pleural space but cannot escape, collapsing the lung and shifting mediastinal structures. The respiration phase follows a clear escalation: seal open pneumothorax, decompress tension pneumothorax, and ventilate when breathing is inadequate. Aligned with CoTCCC and C-TECC guidelines.

Chest Seals Seal

An open pneumothorax (sucking chest wound) occurs when a penetrating injury creates a hole in the chest wall that communicates with the pleural space. Vented chest seals are the preferred intervention -- they occlude the wound on inspiration while allowing trapped air to escape on expiration, preventing the progression to tension pneumothorax. Apply a vented seal to the entry wound and a non-vented seal to any exit wound.

Decompression Needles Decompress

When a pneumothorax progresses to tension pneumothorax, trapped air compresses the heart and great vessels, causing obstructive shock. Needle decompression releases the trapped air by inserting a large-bore needle into the pleural space at the 2nd intercostal space (mid-clavicular) or 4th-5th intercostal space (mid-axillary). The 14-gauge x 3.25-inch ARS is the CoTCCC standard; longer needles may be needed for muscular patients. Chest tubes provide definitive decompression for ongoing air leaks.

Ventilation & Oxygen Ventilate

When spontaneous breathing is inadequate -- due to traumatic brain injury, massive hemorrhage, or airway compromise -- mechanical ventilation becomes necessary. The SAVe II+ automated ventilator provides portable, battery-powered ventilation with set tidal volumes and rates, eliminating the variability of bag-valve mask ventilation during transport. Oxygen concentrators and HME filters support oxygen delivery in austere environments. Capnography (EMMA) confirms tube placement and monitors ventilation effectiveness.

Seal the wound. For any penetrating chest injury, immediately apply a vented chest seal to the entry wound. If an exit wound is present, cover it with a non-vented seal. Vented seals prevent tension pneumothorax by allowing air to escape while preventing air from entering.

Decompress if tension develops. Signs of tension pneumothorax: respiratory distress, absent breath sounds, tracheal deviation, hypotension. Insert a 14G x 3.25" decompression needle at the 2nd intercostal space mid-clavicular line. Listen for a rush of air confirming decompression.

Ventilate when breathing fails. If respiratory effort is inadequate, deploy the SAVe II+ ventilator for consistent tidal volume delivery. Monitor EtCO2 with capnography to confirm placement and guide ventilation rate. Supplement with oxygen as available.
