The Tactical Mechanical Tourniquet™ (TMT™) is a CoTCCC-recommended limb tourniquet developed by Combat Medical (now part of Safeguard Medical). Its 2"-wide band distinguishes it from the 1.5" standard of the C-A-T Gen 7 and SOF® Tourniquet — a meaningful clinical difference: wider bands distribute occlusion pressure over a larger tissue surface area, reducing localized nerve and vessel compression, lowering occlusion pain intensity during prolonged application, and improving performance on proximal thigh wounds where a narrow band may seat less effectively.
The TMT's windlass mechanism combines hook-and-loop closure familiar from the C-A-T with a quick-detach clip reminiscent of the SOF® Tourniquet — delivering a hybrid application sequence that some users find easier to train under combined C-A-T and SOF paradigms. The base plate of the TMT contains two dedicated slots for the TQ-RAM™ point pressure device (sold separately), enabling the TMT to serve as the anchoring compression platform for wound packing maintenance in junctional or proximal injuries where the tourniquet band alone cannot control bleeding. This extended capability is unique to the TMT among all CoTCCC-recommended limb tourniquets.
The Kydex® Rigid Holster included in this bundle is custom-molded to the exact TMT form factor and attaches to belts, plate carriers, and MOLLE webbing via Malice Clips — providing positive, low-resistance extraction in a single pull. The rigid shell protects the TMT windlass mechanism from compression damage and UV/environmental degradation. With a 3.4" × 3.5" × 2" holster footprint and 3.2 oz weight, the complete system adds a well-protected, immediately accessible tourniquet to any operator's loadout. The co-poly resin material of the TMT band includes IR and UV inhibitors for resistance to field storage conditions, and the shelf life extends up to 10 years in operational storage.
How to Apply the TMT

Position — Flick the TMT open and slide or thread it over the injured limb, placing it 2–3 inches above the wound. Ensure the arrow on the strap points toward the patient's heart (proximal).

Cinch — Pull the strap across and through the buckle, removing all slack. No fingers should fit under the band before windlass rotation begins.

Secure the strap — The hook-and-loop section holds the strap in place; confirm no slack remains.

Rotate windlass — Turn the windlass until bleeding stops and no distal pulse is palpable. The quick-detach clip will hold the windlass during rotation.

Lock — Clip the windlass into the retention clip and secure the remaining strap over the windlass.

Document — Write the application time immediately. Reassess after transport or patient position changes.
